Payment policy
At Mid-West Center for Sleep Disorders (MWCSD), we are committed to providing high-quality care and ensuring transparency in our billing and payment practices. This Payment Policy outlines how payments are handled for our services.
1. Accepted Payment Methods
We accept the following forms of payment:
Cash
Personal Checks
Credit/Debit Cards (Visa, MasterCard, Discover, American Express)
HSA/FSA Cards (where applicable)
Payments can be made in person at our office or over the phone.
2. Insurance and Coverage
MWCSD participates with many major insurance carriers. As a patient, it is your responsibility to:
Present a valid insurance card at each visit.
Know your benefits, coverage limits, and referral requirements.
Pay any co-pay, co-insurance, or deductible as determined by your insurance provider at the time of service.
Note: Not all services may be covered by your insurance plan. You are responsible for payment of any non-covered services.
3. Out-of-Pocket Costs
If your insurance does not cover a procedure or service, or if you are uninsured, you are responsible for full payment. We may offer self-pay discounts or payment plans upon request and eligibility.
4. Payment Due at Time of Service
Co-pays and self-pay fees are due at the time of your appointment.
Telehealth services must be prepaid or secured with a card on file.
Failure to make payment at the time of service may result in rescheduling or billing delays.
5. Outstanding Balances
Patients with an outstanding balance may be contacted by our billing office. If a balance remains unpaid after repeated attempts:
Services may be suspended until the balance is resolved.
Delinquent accounts may be referred to a third-party collections agency.
To avoid disruptions in care, please make timely payments or contact us to set up a payment plan.
6. Returned Checks
Returned checks are subject to a $25 service fee. Future payments may be required by cash or credit card.
7. Financial Assistance
MWCSD may offer financial hardship consideration for qualifying patients. Please speak with our billing office to learn more or to request an application.
