At Mid-West Center for Sleep Disorders (MWCSD), we value your trust and strive to provide excellent patient care. This Refund Policy outlines our procedures for cancellations, payments, and refunds related to our services.

1. Appointment Cancellation Policy

We require advance notice for appointment cancellations:

  • In-Person and Telehealth Appointments: Please cancel at least 24 hours in advance.

  • Missed or Late Cancellations: A no-show or late cancellation may result in a cancellation fee, which is non-refundable.

This policy ensures we can offer appointments to other patients in need of care.

2. Payments and Insurance

  • Payments made for covered services through insurance are subject to your insurance provider’s policies.

  • Any co-payments, deductibles, or uncovered services must be paid out-of-pocket and are generally non-refundable unless a billing error occurred.

If you believe a charge was made in error, please contact our billing office promptly.

3. Refund Eligibility

Refunds may be issued under the following conditions:

  • You were billed in error or charged for a service not rendered.

  • Your appointment was cancelled with sufficient notice, and pre-payment was made.

  • A duplicate payment was processed for the same service.

All refund requests are subject to verification and review by our billing department.

4. Non-Refundable Items

The following items are non-refundable:

  • Missed appointment fees

  • Administrative fees

  • Medical record fees (once fulfilled)

  • Services already rendered

5. Refund Process

Approved refunds will be processed within 7–10 business days to the original payment method. If your payment was made via insurance, any refund will follow insurer guidelines and timelines.
